In regard to the data breach, we would like to remind you of important steps to take protect yourself against identity theft or fraud.
 
Identity Theft Affidavit Forms
If you have not done so already, we encourage you to complete and file the following Identity Theft Affidavit documents:

IRS Form 14039 Identity Theft Affidavit

California State Franchise Tax Board Form 3552 (CA only)

 
All Associates Should Complete these Documents
We advise that all associates affected by the data breach should file these documents.
Even if you had already filed your taxes before the breach, we still recommend that you complete this process. Completing this form now will assist with your future tax filing as well.
 
Completing the Documents
To assist you with this process, these forms have been partially completed, and boxes you need to complete have been highlighted.  If a box is not completed or highlighted, you can leave it blank unless it is your personal information. 

Please note that when you file your completed form, you need to attach a copy of one of the following: Passport; Driver’s License or Social Security Card.
 
Submitting these Documents to the IRS
You may submit these documents to the IRS via fax at the below numbers, or mailed in with your tax return. If you are mailing in your tax return, make sure to include these documents.

Change of Address
Please note that the IRS will only communicate via mail which will go to the address last on file. If you moved since filing your last return or after you sent in these documents, please complete the change of address form.

Refund Checks
As a reminder, please do not deposit or cash any refund checks from the IRS unless you are certain they are authentic IRS checks and are in the exact amount that you are expecting.
