Please provide any information available to the commercial dept regarding any incident.  Example for reported leak below:

• Where is the leak coming from?
• When did this happen?
• Do we know if the leak/flood is coming from a residential unit, residential main line, commercial main line, or tenant’s line or hvac?
• Which tenant(s) is leak/flood affecting?
• Do we have any pictures?
• Did we call a plumber to come out and when are they coming?
