In our continued efforts to monitor and manage COVID-19 cases effectively, we will be implementing a Daily Health Survey and COVID-19 Incident Reporting beginning Monday, November 23. Both of these items will be completed in Workday.

WHAT IS THE DAILY HEALTH SURVEY?

• This is a short survey that will be completed by all on-site associates at the start of each shift.
• The survey will be available on the Workday dashboard Both managers and associates will receive alerts to “yes” answers and provided with guidance on next steps.
• Managers will have reporting access to ensure all team members are completing the daily survey.
• This survey can be done on desktop or mobile.

This survey will be rolled out to Corporate Office associates upon re-opening.

WHAT IS COVID-19 INCIDENT REPORTING?

• Managers will be required to report positive COVID-19 cases in Workday, replacing the manual process currently in place.
• The COVID-19 Task Force will be notified and will follow up with with the necessary parties, as needed.
• This will provide easier tracking and reporting capabilities
