This year, associates can get a free Vitality Check biometric screening off-site at a Quest Diagnostics Lab.  The Vitality Check includes four core biometric tests: body mass index (BMI), blood pressure, fasting glucose, and total or LDL cholesterol.

All you need to do is schedule an appointment on Vitality and Quest will submit your results for you! All health results are kept completely confidential.

And best of all - you no longer need to upload your results.

Ready to schedule your Vitality Check biometric screening? Here's what you need to do: 

Log into your Vitality profile
Go to the "Resources" tab
Click on the "Guide to Vitality" tab
Click "Vitality Points" under Vitality Overview
Choose "Vitality Check"
Choose "Schedule your Vitality Check with Quest" under "Quick Links"

After the screening, Quest Diagnostics will send your results to Vitality for you, and your Vitality Points will be awarded in two to four weeks. Quest Diagnostics will also send you a copy of the lab results that you can share with your primary care provider.
