​As the COVID-19 vaccine becomes more widely available, it’s clear that it will be a critical tool in the efforts to help stop the spread of the virus. With that in mind, Essex is excited to offer associates who are vaccinated with an opportunity to earn 500 Vitality Points – 250 for each vaccination dose received!

HOW TO EARN VITALITY POINTS FOR THE COVID- 19 VACCINATION
To earn points for the COVID-19 vaccination, please submit one of the following forms of supporting documentation:

• A completed Prevention Activity form that has been signed by the medical professional who administered your vaccine
• A medical record from your vaccination provider showing that you received the vaccine

You can submit proof of your COVID-19 vaccine online on PowerofVitality.com under Your Account > Forms & Waivers > Prevention.

Alternatively, you can email wellness@powerofvitality.com or fax your documents to (877) 224-7110.

The COVID-19 vaccination is worth 500 Vitality Points. Vaccines that are received in two doses are worth 250 points for each dose. You must submit proof for both doses to receive the full 500 points. Vaccines that are received in one-dose are worth 500 Vitality Points.
